Medical certificate
Medical examination (orzeczenie lekarskie) for professional drivers.
Psychological examination
Psychological certificate (orzeczenie psychologiczne) for professional drivers.
Criminal record certificate
From Poland (zaświadczenie o niekaralności).
If you are from Ukraine — also a Ukrainian certificate of no criminal record plus a sworn translation into Polish.
